By signing this Registration Form, I confirm that to the best of my knowledge, I am physically fit, free from any forms of contagious diseases, emotionally and mentally stable and well enough to participate to participate in all return demonstrations that requires me to undertake.

That I agree and does hereby release from any liability and indemnify and hold harmless the Philippine Red Cross and any of its agency representing or related to the above-mentioned course. This release is for any and all liability for personal injuries (including death) and property losses or damage occasioned by, or in connection with any activity or accommodation for this training. The undersigned further agrees to abide all the rules and regulations promulgated by the Philippine Red Cross.
